NDIS Referral Form

provides funding for supports and services to eligible Australians with permanent and significant disabilities, aiming to improve their quality of life and increase independence.

Medicare

Mental Health Treatment plan (MHTP) allows you up to 10 rebated sessions per year.

Please see your G.P, paediatrician or psychiatrist for a referral letter. You can email us the referral here. Alternatively, bring your referral the first session.
